  • specialises in the provision of dynamic aged rehabilitation services to clients over 65 years old and support them to transition from in-patient hospital care back to the community through an integrated care approach.
  • Rotations through Geriatric Flying Squad- a rapid response multidisciplinary team who provide comprehensive geriatric assessments, treatment and case coordination for older people residing in the community. Aged Care Assessment Team- part of SESLHD Aged Care Assessment Programme, one of four ACAT hubs within the district, offering comprehensive aged care assessment services. Transitional Aged Care – offering a superior multidisciplinary community and residential re-ablement package program. Young Onset Dementia Program- provides a community-based respite service with a coordinated approach to promote health and wellbeing of younger people (45-65years) living with dementia
  • Provides a robust, evidenced based training program with a designated preceptor in your first three months
  • A 2-day hospital orientation and 1-day unit orientation and supernumerary shifts with senior nurses
  • Education and training provided to use and operate the electronic medical record and medication system
  • Formal check in points with performance development plans to ensure personal and professional goal achievement that is inclusive of debriefing and clinical supervision
  • Allocated placement shifts to experience care provision by specialist WMH Community teams and services
